29 CFR 1910.134(c)(1): The employer did not establish and implement a written respiratory protection program with worksite-specific procedures. The program shall be updated as necessary to reflect those changes in workplace conditions that affect respiratory use. The employer shall include in the program the following provisions of this section, as applicable: 1) Procedures for selecting respirators for use in the workplace; 2) Medical evaluations of employees required to use respirators; 3) Fit testing procedures for tight-fitting respirators; 4) Procedures for proper use of respirators in routine work or likely emergencies; 5) Procedures and schedules for cleaning, disinfecting, storing, inspecting, repairing, discarding, and otherwise maintaining respirators; 6) Procedures to ensure adequate air quality, quantity, and flow of breathing air for atmosphere-supplying respirators; 7) Training of employees in the respiratory hazards to which they are potentially exposed during routine and emergency situations; 8) Training of employees in the proper use of respirators, including putting on and removing them, any limitations on their use, and their maintenance; and 9) Procedures for regularly evaluating the effectiveness of the program. On or about 24 May 2016, and at times prior thereto, the employer did not establish and implement a written respiratory protection program for employees conducting spraying operations in the paint booth. Therefore, the following requirements were not completed to ensure employee protection from respiratory hazards. Medical evaluations and fit testing were not conducted prior to requiring the use of respirators. Painters at the site were required to wear air purifying respirators when conducing painting operations in the paint booth. The painters utilized North full-face respirators with organic vapor cartridges.

Extreme care shall be exercised; the block shall be "inched" into the limit or run in at slow speed. If the switch does not operate properly, the appointed person shall be immediately notified. On or about 24 May 2016, and at times prior thereto, crane operators did not perform a function test of the upper and lower limit switches for the approximately 19 overhead cranes located at the facility, exposing the operators to the hazard of being struck by falling pieces from the crane and spool.

On or about 24 May 2016, and at times prior thereto, an employee would access the roof of a mezzanine over the Pallet Build Area to either pull or store bullet heaters, which placed him within four feet of the edge, exposing the employee to a fall hazard of approximately 9 feet to the concrete slab below. Berridge Manufacturing Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.23(c)(1), which was contained in OSHA inspection number 315627323, citation number 01, item number 002 and was affirmed as a final order on 03/21/2012, with respect to a workplace located at 6515 Fratt Road, San Antonio, TX 78218.

On or about 10 May 2016, the employer failed to report the finger amputation of an employee due to a work-related incident within (24) twenty-four hours of the injury event. The injured employee sustained an amputation of the left middle finger up to the second knuckle on 10 May 2016. The work-related incident was not reported to the Occupational Safety and Health Administration until 20 May 2016 by the employer, or approximately 10 days later.
