29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface, and the ladder side rails did not extend at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access:  a. On May 31, 2016, on the lower level roof, the ladder used to access the upper level roof did not extend 3 feet above the landing, exposing employees to a fall hazard.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:  a. Employees performing framing work on the 8:12 pitch roof were not protected from an 18 feet fall hazard by the use of a fall protection system.   b. Employees cutting wood on the 3:12 pitch roof were not protected from a 9 feet fall hazard by the use of a fall protection system.   Framing Crazy Fools was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1111677, citation number 1, item number 1 and was affirmed as a final order on January 4, 2016, with respect to a workplace located at 3622 Paradise Way, Jacksonville Beach, Florida 32250.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stairrail system along each unprotected side or edge:  a. On May 31, 2016, inside the house, the lower level of stairs from the 1st floor to the 2nd level floor did not have a handrail, exposing employees to a 4 feet fall hazard.
