29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:     a. On or about May 4, 2016, on the Timber Mill area at the north side, employee feeding wood into the chain conveyor was injured when his finger was crushed between the chain conveyor and the wood due to lack of guard to keep the employee out of the hazardous area, exposing the employee to crushing and amputation hazard.    b. On or about June 1, 2016, on the Timber Mill area at the south side, employee feeding wood into the chain conveyor was exposed to crushing and amputation hazard due to lack of guard to keep the employee out of the hazardous area.
