29 CFR 1910.132(a): Protective equipment for the eyes was not provided as necessary by reason of chemical hazards encountered in a manner capable of causing injury or impairment in the function of the eyes through physical contact. Print Shop - Eye protection was not provided to the offset press operator exposed to the risks of eye injury resulting from splashes while cleaning the press equipment using squirt bottles containing hazardous chemicals, including but not limited to A230 wash solution, on or about June 6, 2016. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use. (a) Print Shop - An eyewash station was not provided for the press operator who was exposed to the hazard of being splashed in the eyes while conducting the daily cleaning of the offset press equipment using a squirt bottle containing the corrosive chemical A230 wash solution, on or about September 12, 2016. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.
