On or about June 6, 2016, the employer did not ensure the driver of a Hyster, Model S155FT, powered industrial vehicle (forklift) stably or safely arranged a load of three 5045lbs steel coils and exercised caution when handling the off-center load.  The forklift operator placed the load on a single fork that was not centered to the forklift and forks, resulting in the forklift tipping forward and the load falling from the forks.
