29 CFR 1918.65(h)(9): When lift trucks or other mechanically powered vehicles were being operated on open deck-type barges, the edges of the barges were not guarded by railings, sideboards, timbers or other means sufficient to prevent vehicles from rolling overboard:    AML Landing, Barge "Chichagof Provider":  On or about June 13, 2016 and at times prior thereto, during the "pass/pass" stow operation, the waterside edges of the barge "Southeast Trader" being used as a dock was not protected by sufficient means. This condition exposed operators to drowning hazards.

29 CFR 1917.112(a)(1): Vehicle curbs, bull rails, or other effective barriers at least 6 inches in height were not provided at the waterside edges of aprons and bulkheads:  AML Landing:  On or about June 13, 2016 and at times prior thereto, during the stow of the barge "Chichagof Provider", the waterside apron was not protected by curbing, bull rails, or other effective barriers.  This condition exposed employees to drowning hazards.

29 CFR 1917.152(d)(1)(xvi): Oxygen cylinders were not separated from fuel gas cylinders and combustible materials by either a minimum distance of 20 feet (6.1 m) or a barrier having a fire-resistance rating of 30 minutes:  AML Landing, Maintenance Area:  On or about June 13, 2016 and at times prior thereto, greases, fuels, oils and other combustibles were stored adjacent to the oxygen and fuel gas welding equipment.  This condition exposed employees to fire and smoke-inhalation hazards.

29 CFR 1910.1200(f)(6)(i): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the information required by 29 CFR 1910.1200(f)(1)(i) through 29 CFR 1910.1200(f)(1)(v):   AML Landing, Maintenance Area:  On or about June 13, 2016 and at times prior thereto, portable containers (two spray bottles) were not labeled with their contents.  This condition limited employee knowledge of the contents and specific precautions required when using the chemicals.
