29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    (a) McDonalds Restaurant, Front entrance: On or about June 23, 2016, and at times prior, the employer exposed employee to fall hazard of 11-feet, 11-inches from the truck-mounted aerial lift, in that personal fall arrest was not provided to employee installing LED lights on the entrance canopy.      Signal Signs of Georgia, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.453(b)(2)9v), which was contained in OSHA inspection number 690939, citation number 1, item number 1 and was affirmed as a final order on 12/04/2012, with respect to a workplace located at 369 Bracketts Way, Blairsville, GA.
