(a)  On or about June 22, 2016, employees were not removed from cave-in hazards while working outside of the trench shield to install a lateral connection for sanitary sewer in an approximate 7'2" deep trench located on Market St. in Fremont, WI.    (b)  On or about June 22, 2016, employees were not removed from cave-in hazards in an approximate 5'3" deep unprotected trench that was not sloped adequately.

(b)  On or about June 22, 2016, employees capping and attaching tracer wire to a sewer lateral were exposed to cave-in hazards in an approximate 5'3" deep unprotected trench that was not sloped adequately.

29 CFR 1926.652(g)(1)(ii): Shields were not installed in a manner to restrict lateral or other hazardous movement of the shield in the event of the application of sudden lateral loads:    On or about June 22, 2016, a trench shield located on Market St. in Fremont, WI was used in an approximate 7'2" deep trench with approximately 4' of space between the shield and trench wall on both sides, allowing it to move laterally if a trench wall were to cave-in.
