29 CFR 1910.145(c)(3): Safety instruction sign(s) were not used where there was a need for general instructions and suggestions relative to safety measure(s):    (a) Dye House, Standard Running Conditioner:  On or about 6/23/2016, the Dye House equipment used for water- proofing shoe lace material did not have a safety instruction sign stating "Do not place hands in running shoe laces. Use only hand tool to straighten and untangle materials", or equivalent, in the open run area where the injury occurred.

29 CFR 1910.212(a)(3)(iii): Special hand tools for placing and removing material(s) were not provided to permit easy handling of material without the operator placing a hand in the danger zone:  (a) Dye House, Standard Running Conditioner:  On or about June 23, 2016, employees were using their hands to untangle knotted shoe lace cord while it was running on the Dye House equipment, exposing them to entanglement hazards.
