29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefore, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.    On or about June 20, 2016, an employees were exposed to amputation hazards while operating a GANNOmat   0802-VB02 (Cam Machine) and the employer did not install proper guarding to eliminate amputation hazards near the point of operation      Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1904.29(a): A log of all Work-Related Injury and Illnesses (OSHA Form 300), and/or the Summary of Work-Related Injury and Illnesses, (OSHA Form 300-A), and/or the Injury and Illness Report (OSHA Form 301) or equivalent forms were not kept by the establishment:      On or about June 28, 2016, the employer did not maintain OSHA 300s (OSHA Logs), OSHA 300A's (Summaries), and Incident Reports (OSHA 301s or equivalent) of all work-related injury and illnesses for the past three years such as, but not limited to, the following:    a) On or about May of 2016, an employee received a work-related injury involving a cut to the finger that resulted into stitches and days away from work.      b) On or about June 20, 2016, an employee received a work-related injury involving an amputation to the right hand pinky finger tip that resulted into stitches and days away from work      Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
