OSH ACT of 1970 Section (5)(a)(1):  The employer did not furnish employment in a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of getting caught in a wood chipper:     Worksite - On or about 06/22/2016, an employee was removing a log jammed in the feeder hopper of a Vermeer Brush Chipper without following safe shutdown procedures and allowing the blade drum to come to a complete stop, resulting in the amputation of three fingers.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    Worksite - The employer did not create or implement an energy control procedure specific to the Vermeer BC1200XL brush chipper on which an employee suffered amputations of three fingers, while attempting to clear a jam from the point of operation.
