29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury:  A).       On or about 07/06/16, job site - 6th floor of 82-23 Broadway, Elmhurst, NY    Employees installing chillers were performing leak tests on pipes using 250 psi of nitrogen.  A 3" pipe was cut without the air pressure being released and struck an employee standing on a 10-ft A-frame ladder.   The employee fell approximately 6-ft to the lower level.   Procedures were not developed and implemented for performing this task.    ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.
