29 CFR 1910.119(d)(3)(i)(B): Information pertaining to the equipment in the process did not include piping and instrument diagrams:  a) On or about 7/18/2016, In the Acrylonitrile Bulk tank area:    The piping and instrument diagrams (P&IDs) did not accurately depict what was physically existing in place and part of the process.  There were deficiencies such as:     1) There was a valve in line between IC2201 and PI2202, it was not depicted on the P&ID.     2) There was a 3rd, 1 inch capped valve located above BA-37, it was not depicted on the P&ID.     3) There was a valve and nipple used for pressure testing that was in place near PSH2204, it was not depicted on the P&ID.     4) There was a bleed valve and 1 inch line shown near transfer pump #4 on the P&IDs, but was not physically on the system.       5) Two 1.5 inch diameter GA-02A valves were locked shut, but the P&ID did not denote any locking devices.    b) On or about 7/18/2016, In the TDPN Process, Building 2A:  The piping and instrument diagrams (P&IDs) did not accurately depict what was physically existing in place and part of the process.  There were deficiencies such as:     1) A sight glass and pipe diameter reducer were out of order, on the TDPN line.       2) A sample valve and drain were added to the steam/ water line, prior to the TDPN line, it was not depicted on the P&ID.     3) There was a reducer and valve depicted on the P&ID located directly below the tank on the TDPN out feed which was not physically on the system.   c) On or about 7/18/2016, In the MPA Nitrile Process, Building 2:  The piping and instrument diagrams (P&IDs) did not accurately depict what was physically existing in place and part of the process.  There were deficiencies such as:     1) There was a check valve located past FEB3226 associated with the ACN feed line, on the P&ID, but was not physically on the system.     2) There was a valve located prior to Detonator Arrestor F-3220 on the system, but was not depicted on the P&ID.       3) The P&ID indicated the presence of a 2 inch check valveCK-3203, while the system had a vacuum relief valve in place.     4) The P&ID depicts a pressure indicator, PI3203, while the system did not have the PI in place.   d) On or about 7/18/2016, In the Epichlorohydrin Bulk tank area:   The piping and instrument diagrams (P&IDs) did not accurately depict what was physically existing in place and part of the process.  There were deficiencies such as:      1) An in line valve on piping between the bulk tank and pump #3 was locked in the open system as part of a safety system.  The P&ID did not depict the valve being locked open.    Abatement certification is required for this item.
