29 CFR 1926.26: Construction areas, aisles, stairs, ramps, runways, corridors, offices, shops, and storage areas where work is in progress were not lighted with either natural or artificial illumination. The minimum illumination requirements for work areas are contained in Subpart D of this part. a) Main Stairway to second floor - On or about 7-15-16 - Employees were exposed to slip, trip and fall hazards from the total lack of lighting in the main stairway to the second floor, floor landing and immediate second floor area.

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupers for personal protection. a) Torrington Library Job Site - On or about 7-15-16 - Temporary electrical power was being provided through extension cords that were not protected with ground fault circuit interrupters in the system.
