29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. a) 4101 Washington Ave., Newport News, VA 23607: On or about July 12, 2016, at approximately 6:21 p.m., an employee who performed tank cleaning suffered heat exhaustion which resulted in an in-patient hospitalization. The employer did not report the in-patient hospitalization to the Occupational Safety & Health Administration until July 15, 2016. No abatement documentation or certification required.
