29 CFR 1917.118(d)(6): The side rails of ladders did not extend at least 36 inches above the top landing surface:    a) On or about July 15, 2016 at the roof access area, employees were exposed to an approximate 6 foot fall hazard while using the fixed ladder leading from the walkway to the roof.     b)  On or about July 15, 2016 at the basement pit area, employees were exposed to an approximate 8 foot fall hazard while using the fixed ladder.
