29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees.    On or about 20 July 2016, and at times prior thereto, an employee was performing service and maintenance work to free a jam within the Corrugated Roll Former without the machine being locked out or tagged out, exposing the employee to the hazard of being caught in the machine.
