29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a. On or about August 3, 2010, an employee installing plastic sheeting was not protected with a guardrail system, a safety net or a fall protection system when working at/on an unprotected side on the third floor/level. The plastic sheeting was installed vertically, one level above the east side of the containment. b. On or about August 3, 2010, an employee installing and securing plastic sheet was not protected with a guard rail system, a safety net or a fall protection system when working at/on an unprotected side on the fourth floor/level. The plastic sheeting was installed horizontally, two levels above the north side of the containment.

29 CFR 1926.501(b)(4): "Holes." 29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces shall be protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. On or about August 4, 2016, employees were exposed to an uncovered floor hole in the walking working surface west of the containment for the asbestos abatement project. Due to this condition, employees had exposure to a fall hazard of approximately 20 feet.

On or about August 4, 2016, there was an unprotected/uncovered hole beside the containment that was adjacent to the air handling units. Due to this condition, employees were exposed to a tripping or stepping into hazard as the hole was not large enough to fall through.

Covers for floors, roofs, and other walking/working surfaces shall meet. the following requirements: 29 CFR 1926.502(i)(2): All other covers shall be capable of supporting, without failure, at least twice the weight of employees, equipment, and materials that may be imposed on the cover at any one time. On or about August 4, 2016, the employer did not assure that a cover comprised of a single sheet of plywood was capable of supporting twice the weight of employees working adjacent to the hole.

Covers for floors, roofs, and other walking/working surfaces shall meet. the following requirements: 29 CFR 1926.502(i)(3): All covers shall be secured when installed so as to prevent accidental displacement by the wind, equipment, or employees. On or about August 4, 2016, there were three holes with unsecured covers beside the asbestos abatement containments: a 24 inch round hole adjacent to the vertical I-beam pillar, an eleven inch round hole adjacent to the rectangular protruding concrete pad and the rectangular 66 by 32 inch hole adjacent to the west wall of the containment.
