29 CFR 1910.1030(d)(2)(i): Engineering and work practice controls were not used to eliminate or minimize employees exposure:   a) Hospital Waste Dumpster located in the vicinity of the loading area: On or about August 5, 2016 and at times prior thereto; Following surgical procedures, Operating Room personnel are inside dumpsters and segregating waste from grey bags in search of missing surgical instruments from the Ear, Nose, Throat (ENT) set, exposing them to Other Potentially Infectious Material (OPIM) and unfixed bone and tissues. An exposure determination had not been made for entering dumpsters to locate missing surgical instruments.    Note*  Abatement certification IS required for this item.

29 CFR 1910.1030(d)(4)(iii)(B)(1)(iii): Regulated waste was placed in containers which were not labeled or color-coded in accordance with 29 CFR 1910.1030(g)(1)(i):  a) Hospital surgical suites: On or about August 5, 2016 and at times prior thereto; employees are placing surgical tissue from full joint replacements and other orthopedic procedures that meet the definition of Regulated Medical Waste (RMW) into grey plastic bags which then enter into the regular waste stream.  Improper classification and segregation of Regulated Medical Waste (RMW) and Other Potentially Infectious Material (OPIM) could result in healthcare worker exposure such as cuts, lacerations and during handling and disposal.         Note* Abatement certification IS required for this item.
