29 CFR 1926.954(b)(1)(i): Personal fall arrest systems did not meet the requirements of Subpart M of this part in that:  The employer did not ensure that each employee on a working surface (horizontal and vertical surface) with an unprotected side or edge which was 6 feet (1.8m) or more above a lower level was protected from falling by the use of a guardrail system, safety net system, or personal fall arrest system as required by 29 CFR 1926.501(b)(1).  At the site on or about August 5, 2016; employees erecting an RM90 power line transmission tower were not protected from falling while working at heights of 33 feet, exposing employees to fall hazards.

At the site on or about August 5, 2016; employees used body belts as part of a personal fall arrest system while erecting a power line transmission tower, exposing employees to fall hazards of at least 33 feet.

At the site on or about August 5, 2016; the employer did not provide a fall protection training program for employees erecting RM90 power line transmission towers at heights of 33, exposing employees to fall hazards.
