29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by a competent person prior to the start of work and as needed throughout the shift:     On or about August 9, 2016, an employee entering an excavation eight feet deep was exposed to cave in hazards and the employer did not ensure a competent person conducted an inspection of the excavations that included at least one manual soil classification analysis method and ensure the use of a suitable sloping or a protective system.
