29 CFR 1926.502(k)(1): A fall protection plan was not prepared by a qualified person and developed specifically for the site where the residential construction work was being performed and conform to the provisions specified in 1926.502(k)(1) through (k)(10):    On or about July 1, 2016, employees were exposed to fall hazards of greater than six feet while performing roofing activities on a residential roof and a suitable fall protection plan was not developed and documented.

Locke's Roofing and Siding, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 689619, citation number 2, item number 1 and was affirmed as a final order on November 6, 2012, with respect to a workplace located at 1278 Day Street, Galesburg, IL.
