Employees were able to access the point of operation created by the exposed portion of saw blade, exposing employees to amputation injuries. (b) The Tannewitz Band Saw BS15 non used portion of the saw blade was not guarded. Employees were able to access the point of operation created by the exposed portion of saw blade, exposing employees to amputation injuries. (c) The Tannewitz Band Saw BS16 non used portion of the saw blade was not guarded. Employees were able to access the point of operation created by the exposed portion of saw blade, exposing employees to amputation injuries. (d) The Tannewitz Band Saw BS17 non used portion of the saw blade was not guarded. 29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Rotary Thermoforming Machine number 1, Maac 03559, did not have adequate guarding to protect operators from pinch points. The employees were exposed to amputation and/or crushing injuries while performing work near and around the carousel while the machine was in operation. (b) Rotary Thermoforming Machine number 2, Maac 03575, did not have adequate guarding to protect operators from pinch points. The employees were exposed to amputation and/or crushing injuries while performing work near and around the carousel while the machine was in operation. (c) Rotary Thermoforming Machine number 4, Maac 03614, did not have adequate guarding to protect operators from pinch points. The employees were exposed to amputation and/or crushing injuries while performing work near and around the carousel while the machine was in operation. (d) Rotary Thermoforming Machine number 5, Maac 03667, did not have adequate guarding to protect operators from pinch points. The employees were exposed to amputation and/or crushing injuries while performing work near and around the carousel while the machine was in operation. (e) Rotary Thermoforming Machine number 6, Maac 03751, did not have adequate guarding to protect operators from pinch points. The employees were exposed to amputation and/or crushing injuries while performing work near and around the carousel while the machine was in operation. (f) Rotary Thermoforming Machine number 8, Maac 72940, did not have adequate guarding to protect operators from pinch points. The employees were exposed to amputation and/or crushing injuries while performing work near and around the carousel while the machine was in operation. (g) Rotary Thermoforming Machine number 10, Maac 04143, did not have adequate guarding to protect operators from pinch points. The employees were exposed to amputation and/or crushing injuries while performing work near and around the carousel while the machine was in operation. Allied Plastics, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 890637, citation number 1, item number 2 and was affirmed as a final order on May 15, 2013, with respect to a workplace located at 150 Holy Hill Road, Twin Lakes WI 53181.
