29 CFR 1910.212(a)(1): The guarding device was not so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle:  a) Ridg-U-Rak, Inc., machine shop.  On or about August 22, 2016, the guard on the #12 Accu Shear was not designed to prevent the operator from having his hands in the danger zone during the operating cycle.  An employee had his finger amputated when the guard was removed during operation.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours:   a) Ridg-U-Rak, Inc., 120 Lake Street in North East, PA.  On or about August 4, 2016 and at times prior there to, an employee amputation was not reported to OSHA within 24 hours.  The accident and resulting amputation took place on or about July 22, 2016.  The incident was not reported to OSHA until July 28, 2016.
