29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of hazardous energy when employees were engaged in activities such as service and/ or maintenance of equipment.  On or about August 17, 2016, an employee engaged in heavy laundry machine servicing activities in the Washer and Dryer Room was exposed to amputation hazards in that lockout tagout procedures were not being utilized when working on the following pieces of equipment:        a) Frontload Huebsch Dryer (Model- 37CGI. SN-STOK0673B69XA)   b) Frontload UniMac Washer Extractor (Model-UW60PVLXU30001. SN- D607016598)
