29 CFR 1910.1200(h)(1):  Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area.       a)  Facility: On or about August 16, 2016 and at times prior; the employer exposed employees to hazards related to chemicals used on site including but not limited to cleaning chemicals, diesel fuel, oil, and hydraulic fluid without training employees in the required element of the hazard communication standard including those element required under the global harmonization revisions to the standard.

29 CFR 1910.1200(e)(1)(i): A list of the hazardous chemicals known to be present using a product identifier that is referenced on the appropriate safety data sheet (the list may be compiled for the workplace as a whole or for individual work areas); and,  a)  Facility: On or about August 16, 2016 and at times prior thereto the employer exposed employees to chemical hazards in that the employer failed to maintain a list of the hazardous chemicals known to be present at the facility in accordance with the regulation.
