Instance a:  On or about 8/12/2016, at the Blanchard Refining Company LLC located in Texas City Texas, the employer failed to train Decokers on changes through management of change (MOC) MOC-M20144780-003 Coker DCS BACP Project Infrastructure.       Instance b:  On or about 8/12/2016, at the Blanchard Refining Company LLC located in Texas City Texas, the employer failed to inform Decokers of the management of change (MOC) MOC-M20156684-001Remote Coke Cutting- Buildings and Automation Controls Program which included installation of vibration monitor devices to the Coker Units.
