29 CFR 1926.416(a)(1): No employer shall permit an employee to work in such proximity to any part of an electric power circuit that the employee could contact the electric power circuit in the course of work, unless the employee is protected against electric shock by deenergizing the circuit and grounding it or by guarding it effectively by insulation or other means. Located on the south side of a commercial structure, employees utilizing Werner aluminum extension ladders to perform EIFS work were exposed to electrical hazards while working in proximity of uninsulated or deenergized 7,200 voltage power lines. 29 CFR 1926.416(a)(3): Before work is begun the employer shall ascertain by inquiry or direct observation, or by instruments, whether any part of an energized electric power circuit, exposed or concealed, is so located that the performance of the work may bring any person, tool, or machine into physical or electrical contact with the electric power circuit. The employer shall post and maintain proper warning signs where such a circuit exists. The employer shall advise employees of the location of such lines, the hazards involved, and the protective measures to be taken. Located on the south side of a commercial structure, employees utilizing Werner extension ladders to perform EIFS work were exposed to electrical shock while working near 7, 200 voltage energized power lines that were not properly posted with a warning sign. The employees had not been advised by the employer of the location of the electrical lines, hazards involved, and protective measures to take to minimize these hazards. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1926.451(c)(1)(ii): Guys, ties, and braces shall be installed according to the scaffold manufacturer's recommendations or at the closest horizontal member to the 4:1 height and be repeated vertically at locations of horizontal members every 20 feet (6.1 m) or less thereafter for scaffolds 3 feet (0.91 m) wide or less, and every 26 feet (7.9 m) or less thereafter for scaffolds greater than 3 feet (0.91 m) wide. The top guy, tie or brace of completed scaffolds shall be placed no further than the 4:1 height from the top. Such guys, ties and braces shall be installed at each end of the scaffold and at horizontal intervals not to exceed 30 feet (9.1 m) (measured from one end [not both] towards the other). Located on the south side of a commercial structure, employees were exposed to a fall hazard while working from a Fabricated Framed Scaffold not utilizing guys, ties, or braces installed to restrain from tipping. Potential fall height measured to be 20.2 feet.
