29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    (a) Electrical Room and Roof:  On or about, August 25, 2016, employees were exposed to electrical and amputation hazards as lockout tagout was not utilized when unjamming product from the airlock for the baghouse.      NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19

29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):    (a) Roof:  Employees were exposed to amputation hazards as the belt and pulley for the Baghouse Discharge Auger was not guarded as the cover had come off.  Employees were exposed to the unguarded pulley which with the belt creates an ingoing nippoint.

29 CFR 1910.219(e)(3)(i): Vertical or inclined belt(s) were not enclosed by guard(s) conforming to the requirements specified at 29 CFR 1910.219(m) and (o):  (a) Roof:  Employees were exposed to amputation hazards as the vertical belt for the Baghouse Discharge Auger was not guarded as the cover had come off.  The belt and pulley creates an ingoing nippoint which is an amputation hazard.
