29 CFR 1910.147(c)(5)(ii)(C)(2): Tagout devices and their means of attachment were not substantial enough to prevent inadvertent or accidental removal: The employer is failing to ensure that employees are not exposed to forklifts that have been tag out of service. This was most recently documented at 12500 I St. Omaha, Nebraska. The employer was using a tag out system that utilized a thin wire as a form of attachment. The tag system being utilized at the time of the inspection and attached to the CAT Model 2C5000 Serial AT9C250716 was not substantial enough to prevent the inadvertent or accidently removal of the tag.

This was most recently documented at 12500 I St. Omaha, Nebraska. The employer failed to ensure that the red tag being utilized for tag out of the CAT model 2C5000 Serial AT9C35076 identified the hazardous condition that would be present if an employee was to attempt to operate this machine.
