29 CFR 1910.119(f)(4): The employer did not develop and implement safe work practices to provide for the control of hazards during operations such as lockout/tagout; confined space entry; opening process equipment or piping; and control over entrance into a facility by maintenance, contractor, laboratory, or other support personnel: On September 7, 2016 and at times prior thereto, the employer at Marathon Galveston Bay Refinery did not develop and implement safe work practices to control hazards during opening of the discharge piping of the P1008B Depropanizer Reflux Pump as part of placing the pump back in service after being blinded for maintenance work exposing workers to highly hazardous chemicals, including but not limited to hydrogen fluoride and isobutene and propane, which can cause chemical and thermal burns, asphyxia, pulmonary edema, pneumonitis, and other injuries, up to and including death. a) The employer did not implement its Work Permit Retention policy which is part of its Safe Work Permit Procedure to ensure that issued work permits were not discarded. b) The employer did not employ measures to ensure that maintenance craftsmen initial and date Master Isolation Lists to verify proper installation of energy isolation devices they personally installed. c) The employer did not implement a means for periodically checking for and safely releasing hazardous energy in equipment that has been isolated while relying on the Master Isolation List for energy isolation. d) The employer did not ensure that means for checking and safely releasing reaccumulated hazardous energy was included on the Safe Work Permit.

On September 7, 2016 and at times prior thereto and at subsequent times, the employer at the Blanchard Refining Galveston Bay Refinery did not implement written procedures to manage changes to the discharge piping of the P1008B Depropanizer Reflux Pump when the spectacle blind at the flange that joins the pump to the discharge piping was changed out and the replacement blind was not an in-kind replacement. b) The spectacle blind at the discharge of the P-1008B Depropanizer Reflux Pump was changed to a spectacle blind with an integrated bleed feature without implementing the GBR procedure for managing changes to processes.

On September 7, 2016 and at times prior thereto in the Alky 3 Unit at the Marathon Galveston Bay Refinery where there was a possibility of re-accumulation of stored energy in the discharge piping from the P-1008B Depropanizer Reflux Pump, a segment of piping where a line opening was to occur, the employer did not continue to verify the isolation until the maintenance work was completed or possibility of such accumulation no longer existed.
