29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  a) 464 S Sixth St, Fulton, NY 13069, On or about 9/8/16:  The employer failed to provide proper Fall Protection to its employees while performing roofing activities on a 6/12 pitch roof that had a fall distance of 16 feet 4 inches from the eaves edge  to the ground below.     The employer, Robert O'Brien, dba O'Brien Home Remodeling Services  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1005405, citation number 1 , item number 2 and was affirmed as a final order on 1/6/2015 with respect to a workplace located at 324 Kensington Rd, Syracuse, NY 13210.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.  a) 464 S 6th St, Fulton NY, On or about 9/8/16: The employer failed to  ensure that a ladder being used to access the roof area was not extended  three feet above the landing surface to afford employees a safe transition from the ladder to the landing surface.  The employer, Robert O'Brien, dba O'Brien Home Remodeling Services  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1) which was contained in OSHA inspection number 1005405, citation number 1 , item number 3 and was affirmed as a final order on 1/6/2015 with respect to a workplace located at 324 Kensington Rd, Syracuse, NY 13210.
