29 CFR 1904.29(a): Basic requirement. You must use OSHA 300, 300-A, and 301 forms, or equivalent forms, for recordable injuries and illnesses. The OSHA 300 form is called the Log of Work-Related Injuries and Illnesses, the 300-A is the Summary of Work-Related Injuries and Illnesses, and the OSHA 301 form is called the Injury and Illness Incident Report.  Located at Superior Production Partnership LLC dba Superior Die Tool and Machine Company in Columbus, Ohio:  a.  On the 2016 OSHA 300 Log the employer did not provide all information required in Column F such as the object or substance that directly injured or made the person ill, the actual injury description, and body part affected.       1.  Case 16-041F, column F stated, "Laceration: Left Thigh", which is insufficient to determine what caused the injury.       2.  Case 16-042F, column F stated, "Contusion: Left Knee", which is insufficient to determine what caused the injury.       3.  Case 16-043F, column F stated, "Laceration:  Stomach", which is insufficient to determine what caused the injury.       4.  Case 16-044F, column F stated, "Laceration:  R-back leg", which is insufficient to determine what caused the injury.         5.  Case 16-046F, column F stated, "Contusion: Left Middle Finger", which is insufficient to determine what caused the injury.       6.  Case 16-047F, column F stated, "Broken Left Hand Thumb", which is insufficient to determine what caused the injury.     7.  Case 16-045F, column F stated, "Laceration:  back of the head", which is insufficient to determine what caused the injury.     8.  Case 16-048F, column F stated, "Overexertion:  Left side of Neck", which is insufficient to determine what caused the injury.     9.  Case 16-049F, column F stated, "Left Hand: Struck/Pinched", which is insufficient to determine the actual injury and what caused the injury.     10.  Case 16-050F, column F stated, "Left Wrist: Overexertion", which is insufficient to determine what caused the injury.     11.  Case 16-051F, column F stated, "Laceration: Left finger", which is insufficient to determine what caused the injury.     12.  Case 16-052F, column F stated, "Contusion: Right Wrist", which is insufficient to determine what caused the injury.     13.  Case 16-053F, column F stated, "Puncture: Left Arm", which is insufficient to determine what caused the injury.     14.  Case 16-054F, column F stated, "Puncture: Right Thumb", which is insufficient to determine what caused the injury.     15.  Case 16-055F, column F stated, "Puncture: Left Finger", which is insufficient to determine what caused the injury.     16.  Case 16-056F, column F stated, "Contusion: Thumb", which is insufficient to determine what caused the injury and which thumb was injured.     17.  Case 16-057F, column F stated, "Corneal Foreign Body", which is insufficient to determine the actual injury, which eye was injured, and type of foreign body that caused the injury.     18.  Case 16-058F, column F stated, "Sprain-R-Elbow", which is insufficient to determine what caused the injury.     19.  Case 16-059F, column F stated, "Open wound of shoulder right", which is insufficient to determine the actual injury and what caused the injury.     20.  Case 16-060F, column F stated, "Corneal Foreign Object Eye, which is insufficient to determine the actual injury, which eye was injured, and type of object that caused the injury.     21.  Case 16-061F, column F stated, "Right Knee: Pain", which is insufficient to determine the actual injury and what caused the injury.     22.  Case 16-062F, column F stated, "Overexertion:  Back of Neck", which is insufficient to determine what caused the injury.     23.  Case 16-063F, column F stated, "Caught Between: L-Hand", which is insufficient to determine the actual injury and what caused the injury.     24.  Case 16-064F, column F stated, "Caught Between: Left Ring Finger", which is insufficient to determine the actual injury and what caused the injury.  b.  On the 2016 OSHA 300 Log the employer did not provide sufficient information in Column E to determine the specific location where the event occurred, such as (but not limited to) a machine or equipment name or a particular area on the production floor.       1. Cases 16-041F through 16-048F, 16-050F, 16-051F, 16-054F through 16-059F, 16-061F, and 16-063F, column E stated, "Production", which is insufficient to identify a specific location where the injury event occurred.     2.  Cases 16-052F, 16-060F, and 16-064F, column E had no information provided.     c.  On the 2016 OSHA 300 Log the employer did not provide information in Column C for the injured employee's job title.      1.  Case 16-052F, column C had no information provided.  d.  On the 2016 OSHA 300 Log the employer did not classify each case using columns G, H, I, and J in accordance with the most serious outcome associated with the injury or illness that occurred.      1.  Cases 16-041F through 16-046F, 16-048F through 16-056F, 16-058F, 16-059F, and 16-061F through 16-063F did not have a classification provided.
