29 CFR 1910.23(a)(3)(ii): Where operating conditions necessitate the feeding of material into any hatchway or chute opening, protection shall be provided to prevent a person from falling through the opening: On or about September 13, 2017, at or near 102 South 13th St., Mermentau, LA; employees fed material into a hatchway or chute opening. Protection was not sufficient to prevent a person from falling through the opening and contacting a moving auger below.
