29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:    (a) On or about September 22, 2016 - Outside loading area, the employer allowed employees to attach a safety platform to the forks of a Yale forklift truck without prior written approval from the forklift manufacturer.
