No abatement certification or documentation required.

29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: (a) GRINDER ROOM - Employees performed service on the Cresswood Grinder, and each authorized employee involved in the process did not affix a personal lockout device to the lockout device prior to working on the equipment, which thereby exposed the employee(s) to serious injury, on or about November 10, 2016. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

No abatement certification or documentation required.
