29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment was not physically located and operated in such a manner as to isolate the machine or equipment from the energy source:    (a)  DISMANTLER ROOM - Employees changed the blades on the Run-a-gade Band Saws (D1, D2, D3), and the employees did not de-energize the equipment using the electrical disconnects, which thereby exposed the employees to serious injury, on or about September 21, 2016.    No abatement certification or documentation required.

29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment:  (a)  GRINDER ROOM - Employees performed service on the Cresswood Grinder, and each authorized employee involved in the process did not affix a personal lockout device to the lockout device prior to working on the equipment, which thereby exposed the employee(s) to serious injury, on or about November 10, 2016.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    (a)  DISMANTLER ROOM - Employees dismantled pallets using horizontal band saws, and the band saws were not effectively guarded to prevent employees from placing their hands into the danger zone of the saws, thereby exposing the employees to the running saw blade where the employees routinely placed their hands within 5 to 8 inches of the blade, on or about September 21, 2016.     Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven 7 feet or less above floors or platforms were not enclosed:    (a)  DISMANTLING AREA - The employer did not ensure that the chain and sprocket drive for the conveyor that removes dismantled pallet debris from the horizontal band saw area was fully enclosed, which thereby allowed an employee to stick his hand into the chain and sprocket area and sustain amputation injuries, on or about September 16, 2016.    No abatement certification or documentation required.
