29 CFR 1926.453(b)(2)(iv): The employer did not ensure that employees stand firmly on the floor of the basket, and shall not sit or climb on the edge of the basket or use planks, ladders, or other devices for a work position.  This violation occurred on October 5, 2016, where at least one (1) employee was working on an aerial lift without fall protection and was exposed to a fall of approximately 18 feet to the concrete below.
