29 CFR 1910.36(d)(3):  Exit route doors were locked from the inside not only in mental, penal, or correctional facilities.  The west exit door was locked from the inside.   Abatement certification and abatement document is required for this violation.  The documentation should include written verification of abatement, applicable measurements, or monitoring results, and photographs or videos, which you believe will be helpful.  The abatement certification sheet is enclosed with the notice(s).

29 CFR 1910.37(a)(4): Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times:  (a)  In the hall near the bathrooms, the employer did not ensure that the route was adequately lighted in the event of a power outage as the only illumination battery back up lighting , when tested using the test switch, did not operate.    (b)  In the bathrooms, the employer did not ensure that the route was adequately lighted in the event of a power outage as the only illumination battery back up lighting , when tested using the test switch, did not operate.
