29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  On or about 10/5/16 at the construction site located at 606 Mane Street, West Monroe, LA; the employer allowed two employees to work from an aerial lifts at the front of the building without wearing any full body harnesses attached to the baskets; employees were exposed to falls of 33' and 47' above the ground and accompanied bone fractures and other injuries.
