29 CFR 1910.178(q)(6): Industrial trucks were altered so that the relative positions of the various parts were different from what they were when originally received from the manufacturer, or they were altered either by the addition of extra parts not provided by the manufacturer or by the elimination of parts:  109 Cadwell Drive, Springfield, MA  On October 13, 2016 and at times prior, employees operated two powered industrial trucks without using seat belts due to the fact that one half of the two-part seat belt system was missing from the powered industrial trucks.

MA    On October 13, 2016 and at times prior, welding stations were not fully screened and employees were not required to wear goggles while working adjacent to the welding area or while operating powered industrial trucks in the area.
