29 CFR 1926.28(a): Appropriate personal protective equipment was not worn by employee(s) in all operations where there was exposure to hazardous conditions:    (a) On or about 10/6/2016 - at 7200 Jefferson Metropolitan Parkway, McCalla, AL, employees were exposed to electrical hazards such as but not limited to arc flash due to not wearing arc flash personal protective equipment and installing a bracket at the Main Distribution Panel Switchboard section 3 that was energized with 480V.

29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:     (a) On or about 10/6/2016 - at 7200 Jefferson Metropolitan Parkway, McCalla, AL, employees were permitted to install a bracket on the top of the Main Distribution Panel Switchboard section 3, 480V, while the circuit was energized, the top front panel was removed, and guarding by insulation or other means was not provided.
