29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: Second floor NW corner: An employee was exposed to a fall hazard greater than 10 feet to the concrete floor below while performing the task of installing material lift anchors. The employee was operating a Skyjack Scissor lift, Model SHIL 3219 to access the work location while the scissor lift's midrail access chain was not attached.

The employee was operating a Skyjack Scissor lift, Model SHIL 3219 to access the work location while the employees Personal Fall Arrest System (PFAS) was attached to the guardrails of this lift.
