29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    (a) American Heritage Painting, at 2525 16th Street, Greeley, CO: On 10/31/16, the employer did not ensure that each employee working on a steep roof with an unprotected side or edge which was six feet or more above a lower level was protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system.  Employees of American Heritage Painting were repairing and replacing sections of a wood shake shingle roof on a two story commercial building with a pitch over 4/12.  The employees were not protected from a fall.  This condition exposed the employees to an approximate fifteen foot fall hazard.

Employees of American Heritage Painting were repairing and replacing sections of a wood shake shingle roof on a two story commercial building.  The employees accessed the roof via an aluminum extension ladder.  The side rails of the extension ladder did not extend past the roof edge.  This condition exposed the employees to an approximate twenty-one foot fall hazard.

Employees of American Heritage Painting were repairing and replacing sections of a wood shake shingle roof on a two story commercial building.  The employees accessed the roof via an aluminum extension ladder.  The feet of the extension ladder were placed upon approximately two to three inch diameter round rocks in a rock bed.  This condition exposed the employees to an approximate twenty-one foot fall hazard.
