On 11/01/2016, an employee operating a motorized pallet jack struck the forks the of a parked forklift truck located in an opposing travel position, causing the employee to be thrown from the pallet jack.  In addition, designated pedestrian walkways were not provided.    Condition observed on or about 11/8/2016
