29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer report the in-patient hospitalization, amputation, or loss of an eye to OSHA.     A. Chaney Roofing and Maintenance worksite located at 139 Buckeye St., Port Clinton, Ohio: On or about October 28, 2016, the employer failed to report an employee hospitalization (admittance) following a worksite incident (fall from elevated work surface).  The employer or employer representative failed to contact the Toledo Area Office or the 24 hour OSHA number to notify the agency of employees hospitalization within 24 hours of admittance. The injured employee was admitted on October 28, 2016, for related treatment.  The employer failed to contact OSHA between the time of admittance and the referral received by the Toledo AO on November 8, 2016.
