29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  a) Roberto Mendoza on our about November 16, 2016 at the address of 4864 Overland Parkway Toledo OH: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards, such as falls during jobsite operations, decreasing the probability of employee injury and/or illness.   Roberto Mendoza was previously cited for a violation of this occupational safety and health standard which was contained in OSHA Inspection Number 1145294, citation number 1,  item number 1a, and was affirmed as a final order on 6/9/2016, with respect to a workplace located at 1233 Cribbs Street, Toledo, Ohio.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) Roberto Mendoza on our about November 16, 2016 at the address of 4864 Overland Parkway Toledo OH: The employer did not ensure employees were adequately protected from falls while performing pre framing activities atop a residential type structure. Employees tasked with setting wood trusses were exposed to falls of approximately 18 feet the ground below.  Roberto Mendoza was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA Inspection Number 1145294, citation number 1,  item number 1b, and was affirmed as a final order on 6/9/2016, with respect to a workplace located at 1233 Cribbs Street, Toledo, Ohio.
