29 CFR 1910.179(b)(3): Crane(s) were modified and such modifications and the supporting structure were not checked thoroughly for the new rated load by a qualified engineer or the equipment manufacturer:  (a) Hoist 2: On November 17, 2016 and at times prior, employees were exposed to struck by hazards while working with a JET 1 Ton manual chain hoist mounted on a fabricated structure. The employer failed to have the crane and structure assembly rated by a qualified engineer or the equipment manufacturer.  (b) Hoist 22:On November 17, 2016 and at times prior, employees were exposed to struck by hazards while working with a CM Loadstar 1/2 Ton electric chain hoist mounted on a fabricated structure. The employer failed to have the crane and structure assembly rated by a qualified engineer or the equipment manufacturer.   (c) Hoist 24: On November 17, 2016 and at times prior, employees were exposed to struck by hazards while working with a Chester 1 Ton manual chain hoist mounted on a fabricated structure. The employer failed to have the crane and structure assembly rated by a qualified engineer or the equipment manufacturer.

29 CFR 1910.179(j)(3): A complete periodic inspection of crane(s) had not been conducted in the past 12 months:  (a) Hoist 1: On November 17, 2016 and at times prior, employees were exposed to struck by hazards while working with a Log Truck Trailer Cable hoist that did not have an annual inspection completed.  (b) Hoist 2: On November 17, 2016 and at times prior, employees were exposed to struck by hazards while working with a JET 1 Ton manual chain hoist that did not have an annual inspection completed.  (c) Hoist 4: On November 17, 2016 and at times prior, employees were exposed to struck by hazards while working with a CM Loadstar 1/2 Ton Electric chain hoist that did not have an annual inspection completed.  (d) Hoist 5: On November 17, 2016 and at times prior, employees were exposed to struck by hazards while working with a JET 1 Ton electric chain hoist that did not have an annual inspection completed. (i) Hoist 10: On November 17, 2016 and at times prior, employees were exposed to struck by hazards while working with a JET 3 Ton electric chain hoist that did not have an annual inspection completed. (p) Hoist 17: On November 17, 2016 and at times prior, employees were exposed to struck by hazards while working with a Dayton 1 Ton electric chain hoist that did not have an annual inspection completed.
