29 CFR 1910.179(g)(1)(v): Pendant control box(es) were not clearly marked for identification of functions:  On or about November 30, 2016, at the above addressed site, an employee was exposed to struck and caught by hazards when using a J. Herbert 10 ton overhead crane while the pendant control box did not have the push buttons clearly marked as to their function.

29 CFR 1910.179(j)(2): The following items were not be inspected for defects at intervals as defined in paragraph (j)(1)(ii) of this section or as specifically indicated, including observation during operation for any defects which might appear between regular inspections:  On or about November 30, 2016, at the above addressed site, an employee was exposed to struck caught by hazards from  an overhead crane that was not frequently inspected to identify defects, including but not limited to a damaged and non-functional throat latch on the hook.
