The employer did not ensure that the metal frame scaffold whose height to base width ration exceeded 4:1 was restrained.   Employees were working on a scaffolding that was over 50 foot in height and was improperly restrained from tipping with improperly installed anchors.

The scaffold frames on the fifth and sixth levels were bent and the cross bracing was not connected at multiple locations throughout the scaffolding.
