29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):    a) Formula Roofing at 1414 Birch Street Denver, CO 80220: On or before 11/29/2016, the controlling employer did not ensure that sub-contractor employees were using fall protection when roofing a ranch-style single family home with up to a 5/12 pitch.  The sub-contractor Employees were using pneumatic nail guns to shingle and were not wearing harnesses and were not tied off.  No other fall protection was in use.
